I wish to submit the images listed below for consideration for inclusion in the OPS annual exhibition. I confirm that the images submitted are exclusively my own work and that I hold the sole copyright for each of the images listed. I also confirm that I have in my possession appropriately signed model release forms for all submitted images which include identifiable people in circumstances other than public places, or people under the age of 18. I have read and understood, and confirm that I now accept, the conditions for the submission of work to the Society. I also agree to abide by the rules of the Society and actively support its objectives at stated below.
